Thank you for taking the time to download our form. Before you arrive for your first appointment, please remember to bring with you the following.
- Download, print and complete the registration form.
- Make a list of medications, supplements and herbs you are taking and dosage.
- Gather any cardiac related medical records and testing results.
- Have your photo ID and insurance cards.
- Have your co-pay or co-insurance. We accept all major credit cards, cash and checks.
